If You Need Medications...

If possible, please call us during office hours to have your prescriptions refilled. As soon as your physician has an opportunity to review your medical record, our staff will call your pharmacist with your refill request. During evening hours or on weekends, it is difficult for the "on call" physician to determine if a prescription refill is indicated, as he is without access to your medical record.

If You Have an Emergency...

In the event of a life-threatening emergency, call 911. In the event of an urgent situation during evening hours or on weekends, please call the hospital switchboard at 906-228-9440 or 1-800-562-9753 and your message will be forwarded to the "on call" surgeon who will make every effort to respond to your emergency needs promptly.

If Our Physicians are unavoidably delayed...

Surgical Associates physicians play a major role in the emergency general surgical attending coverage for Marquette General Health System and Bell Memorial. The physicians of Surgical Associates are committed to providing timely care for patients requiring surgical services and to seeing emergency and urgent cases as soon as possible. As a result, there may be occasions when our physicians fall behind in their office schedules either due to unexpected emergencies or extended surgical procedures. Should an unavoidable delay occur, our staff will make every effort to keep scheduled patients notified. If you are unable to wait for your physician, the office will reschedule your appointment for a time that is more convenient. Be sure to ask our receptionist if your appointment is delayed more than thirty minutes to check on your appointment’s status.

How Can We Help Simplify the Payment Process...

Surgical Associates participates with all major insurance carriers including, but not limited to, Blue Cross (in and out of State), Aetna, Cofinity (PPOM), Cigna, United Healthcare, Tricare (Prime & Standard), Wausau, Medicaid (UPUP-MCAC), and Medicare (HMO contracts/Humana-Medicare Advantage). Should your insurance carrier require a referral form, we request that you obtain it from your primary care physician or insurance carrier prior to your visit. Please provide Surgical Associates with complete insurance information on our "Patient Registration" form. At the time of your first office visit, we request that your insurance card be available so that it may be photocopied by our staff. To help us maintain accurate records, please notify our staff of any changes in your address, telephone, or insurance carrier by calling our business office at (906)225-3853. Each time you visit Surgical Associates you may be requested to read and sign a "Payment Policy & Agreement” form". These forms will facilitate the payment of our bills with your insurance company. As a service to our patients, our staff will complete the necessary insurance forms and submit them for you. Payment of required deductibles and co-payments are expected when services are rendered. We accept most major credit cards, cash, and personal checks. If you are uninsured, or experiencing financial difficulty, please contact one of our billing specialist at 906-225-4510, who will gladly provide cost estimates for various services, determine insurance coverage and discuss financial arrangements that are suited to your budget.

Please remember that your insurance policy is a contract between you and your carrier. It is ultimately your responsibility to understand the provisions of your policy and to assure payment for services rendered to you.
